Title: Bilingual (Spanish) EMPOWR Intensive Care Coordinator (ICC) Classification: Regular Full Time (1.0 FTE), Non-exempt (Hourly) Location: Oakland, CA / Hybrid (availability to provide client services in-person and complete documentation remotely) Regular Work Schedule: Full time, M-F Compensation: $34.37 per hour (Rate inclusive of 10% Spanish language differential) The Team-Based model of care program named EMPOWR (EMbracing, Progress and Opportunities, We Rise) aims to support transition aged youth (TAY, 16-24) attain mental health support, housing stability, employment and education gains, and community connections. The program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on one shared caseload, providing youth with an entire team of support, and allowing staff to share in the successes and challenges of their clients. The Bilingual Intensive Care Coordinator (ICC) provides support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth’s natural supports together. This position is responsible for engaging and assisting foster youth and other clients, family members, foster parents, and other support persons in forming a Child and Family Team (CFT). The CFT works with identified support persons from natural supports (friends, extended family, neighbors, community members) and formal system partners (child welfare, education, juvenile probation, etc.) to comprehensively address the child and family’s identified goals. The position draws on lived experience to provide support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth’s natural supports together. Because of their lived experience, they help create a connection with family and other close contacts, and offer a unique perspective to the rest of the team. How you'll make an impact: Outreach to and engagement with youth; being a strong advocate for the youth within the team Provide community-based trauma-informed services to foster youth or former foster youth transitioning to or struggling with independent living. Provide intensive skill building with youth, with a focus on independent living. Provide assessments, crisis intervention, stabilization, and safety plans. Identify and connect youth & young adults with other agencies to address their needs, including housing, medical, mental health, education, employment, advocacy, placement stability, and enrichment activities. Work collaboratively in partnership with the team to engage and connect clients to appropriate resources and services. Assist with initial contact and intake, establishing relationships and building trust in order to help clients navigate systems for access to services. Provide advocacy, targeted case management and community based rehabilitation services to children and their families. Provide services and participate in agency meetings in a combination of environments including virtual (zoom / phone) and in-person office or community locations (drop-in center, client homes, school, parks, etc.) Provide stabilization support services to children and TAY in an in-person milieu environment.
