About the Role

The Personal Service Coordinator is responsible for coordinating the needs of clients and families assigned to their caseload. The role ensures individualized care planning, facilitates access to community resources, and monitors the effectiveness of services. PSCs provide professional support through client contact, guidance, and instruction, maintaining documentation and compliance with program and contractual requirements.

Responsibilities

Personal Service Coordinator · Conduct intake and complete comprehensive evaluation of clients’ life situations · Prepare individualized plans of care in collaboration with clients · Facilitate access to community resources and monitor participation in services · Provide professional care through regular client contact, including home visits · Maintain accurate daily records, progress notes, and billing forms per program and Short Doyle/Medi-Cal standards · Develop and maintain professional working relationships with agencies and service providers · Adhere to HIPAA standards, ethical standards, and professional boundaries · Transport clients as necessary using personal vehicle and follow program guidelines · Participate in after-hours phone response duties as assigned · Perform other duties as required to support department and program goals Education and Licensing Requirements PSC :
