Competencies: Conducts an individualized patient assessment, prioritizing the data collection based on the patient’s immediate condition or needs within time specified by facilities policy and procedures or protocols. Notifies providers of abnormal findings. Conducts ongoing assessments as determined by the patient’s condition and/or the facility’s policies & procedures or protocols and prioritizes care accordingly. Develops and updates resident care plans that is individualized and reflect collaboration with other members of the healthcare team. Performs appropriate treatments as ordered by providers in an accurate and timely manner. Perform all assigned tasks accurately. Provides individualized patient/family education. Comprehensive documentation of patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution. Answers phone calls, assist the public, and forwards appropriately, if necessary, in a professional and friendly manner. Initiates emergency resuscitative measures according to appropriate resuscitation protocols. Maintains confidentiality in matters related to residents, family, and facility staff.
