The LPN is responsible for client care and the dispensing of controlled substances such as Methadone and Buprenorphine based on physician orders. They provide support to medical staff and case management to help clients achieve positive behavioral changes.
Candidates must be a Licensed Practical Nurse in Arizona and possess a valid Level 1 Fingerprint Clearance Card. Additional requirements include being at least 21 years old with a valid AZ driver's license and a clean driving record.
Possess a valid Arizona Level one Fingerprint Clearance Card, and: Currently a Licensed Practical Nurse (LPN) through the Arizona Board of Nursing. An unacceptable driving record includes (but is not limited to) the following: more than two [2] moving violations in the last three [3] years or; more than one [1] at-fault accident within the last three [3] years or; any major violation (i.e. DUI, Careless/Reckless Driving, Drag Racing, Hit and Run) within the last three [3] years. Other driving related violations may be considered unacceptable. At time of job offer, submission of 39 month (three year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees. Applicants must be able to pass a pre-employment drug screen.
