The IBHH Care Coordinator is part of a team accountable for an individual’s total care, deploying a multidisciplinary, team-based approach to address physical, behavioral, and social health needs using a single Health Action Plan. Services are highly individualized for complex and chronic conditions, utilizing health information and data through technology, delivered via face-to-face, telephonic, or electronic means tailored to maximize engagement.
include a Bachelor's or Master's degree with no experience, or an Associate's degree plus two years of behavioral health experience, or a High School Diploma/GED plus a Peer Support Specialist certification and three years of experience, or a High School Diploma/GED plus four years of behavioral health experience. Applicants must also be 21, possess a valid AZ driver's license with an acceptable record, and pass a drug screen and background check for this safety-sensitive position.
