The Care Coordinator is responsible for rooming patients, documenting vitals, and performing clinical tasks such as specimen collection and medication reconciliation. They also provide patient education and administrative support to ensure smooth clinic operations.

Position requirements - What you need to know before applying
Full-Time Commitment: This is a full-time position (Monday–Friday, 40 hours per week).
Shift Availability: Shifts may begin as early as 7:30 AM and may end as late as 5:30 PM. You must be available to work shifts within this range.
Valid Driver's License & Reliable Transportation: You must have a valid driver's license & reliable transportation to travel locally to our satellite offices around I-270. Mileage reimbursement is available for eligible midday travel.
