Provide relationship-driven, value-based primary care to a senior patient population in an outpatient setting. Manage a small patient panel focusing on preventative care and chronic condition management with support from a dedicated care team.
Must be an MD or DO who is Board Certified or Eligible in Family, Internal, or Geriatric Medicine. Requires an active, unrestricted medical license and DEA license; residents and visa candidates are not eligible.
