POSITION OVERVIEW Heartline Staffing is seeking a patient-focused Physical Therapist for Vibrant Care Rehabilitation in Elk Grove, CA , California. This full-time outpatient rehabilitation opportunity is ideal for a clinician who can independently evaluate patients, develop individualized treatment plans, supervise support staff, and deliver high-quality care in a collaborative, energetic environment.

The Physical Therapist will manage an average caseload of approximately 14 patients per day while providing evidence-based treatment designed to restore mobility, improve function, reduce pain, and support long-term quality of life. This role requires strong clinical judgment, accurate documentation, effective communication, and the ability to work closely with physicians, case managers, families, Physical Therapist Assistants, and rehabilitation aides.

Perform comprehensive musculoskeletal examinations and diagnostic evaluations.

Identify patient needs and determine medical necessity for physical therapy services.

Develop individualized, evidence-based treatment plans.

Provide manual therapy, therapeutic exercise, neuromuscular re-education, and appropriate treatment modalities.

Monitor patient progress and modify treatment plans based on clinical response and functional outcomes.

Maintain an average caseload of approximately 14 patients per day.

Direct and supervise Physical Therapist Assistants and Physical Therapy Aides.

Ensure delegated services remain compliant with the California Physical Therapy Practice Act.

Complete initial evaluations, daily treatment notes, progress reports, and discharge summaries accurately and on time.

Maintain complete clinical documentation within the electronic medical record system.

Communicate with physicians, case managers, family members, and other members of the care team.

Coordinate care for Workers’ Compensation patients and support return-to-work planning when applicable.

Participate in discharge planning and ensure continuity of care.

Contribute to a positive, collaborative, and patient-centered clinical environment.

Maintain safe treatment practices and proper use of clinical equipment. COMPENSATION

Requirements

  • Required qualifications
  • Graduate of an APTA-accredited Physical Therapy program. REQUIRED
  • Active California Physical Therapist license or current California license-applicant status. REQUIRED
  • Current CPR certification. REQUIRED
  • Ability to perform musculoskeletal examinations and diagnostic evaluations. REQUIRED
  • Ability to develop and implement individualized physical therapy treatment plans. REQUIRED
  • Clinical competence in manual therapy, therapeutic exercise, neurological re-education, and treatment modalities. REQUIRED
  • Ability to supervise Physical Therapist Assistants and Physical Therapy Aides in compliance with California regulations. REQUIRED
  • Ability to manage an average caseload of approximately 14 patients per day. REQUIRED
  • Experience completing accurate and timely electronic medical record documentation. REQUIRED
  • Strong clinical problem-solving and decision-making skills. REQUIRED
  • Ability to work effectively within a multidisciplinary care team. REQUIRED
  • Strong written and verbal communication skills. REQUIRED
  • Ability to lift up to 50 pounds. REQUIRED
  • Ability to perform frequent bending, standing, reaching, and other physical demands throughout the workday. REQUIRED

Preferred Qualifications

  • Doctor of Physical Therapy degree.
  • Outpatient orthopedic rehabilitation experience.
  • Workers’ Compensation rehabilitation experience.
  • Experience with ergonomic assessments.
  • Experience performing Functional Capacity Evaluations.
  • Experience supporting return-to-work programs.
  • Bilingual English and Spanish skills.
