Provide social work services to patients with acute, chronic, or terminal illnesses in an acute care setting. Responsibilities include psychosocial assessments, discharge planning, and coordinating with a multidisciplinary team to manage patient throughput.
Requires a Master's degree in Social Work from a CSWE-accredited school and a current Missouri Social Work license (LMSW or LCSW). Prior experience in a healthcare setting is preferred, along with strong communication and computer skills.
Saint Luke's Hospital is seeking a part-time, weekend Social Worker to join our Care Progression team! Part time; in person, Saturday & Sunday: 7am - 7pm Will be working with a Social Work/RN care coordinator team of 5 both days and will need to be able to prioritize their day to best meet throughput. Our weekend team often discharges 40-50 patients on a weekend, so prioritization and critical thinking are a must. Provides social work services to patients in an acute care setting for patients with acute, chronic or terminal illnesses and their families. Services to patients and their families include identification of psychosocial needs, psychosocial assessment, discharge planning, crisis intervention, information and referrals, protective services, and patient/family education.
Master’s degree in social work from a school of social work accredited by the Council on Social Work Education (CSWE) Current Missouri Social Work license (LMSW or LCSW) Knowledge of theory and practice of social work at the MSW level and have basic research skills Must have the ability to work as part of a multidisciplinary team and with patients of various ages and cultural backgrounds Must have excellent written/verbal communication skills Computer skills, Basic typing skills Prefer prior experience in a healthcare setting (i.e., hospital, home health or nursing home) but not required.
