Qualifications

Graduate of an accredited vocational school of nursing Current LPN license (or equivalent) Previous experience in Long Term Care, Home Health, acute care nursing, or Hospice (preferred) Ability to collaborate with others Strong communication and human relation skills CPR certification Written and verbal communication skills Valid drivers license (required) Computer literacy

Responsibilities

Perform patient visits under the supervision of an RN Case Manager Provide patient care as delegated by the RN Monitor nursing care provided by other personnel Assist with data collection and plan of care development Utilize iPad for client documentation Adhere to clinical nursing practice standards and the Nurse Practice Act
