Conduct initial client assessments and continuously refine care plans to ensure patient comfort. Coordinate care through hands-on nursing, patient education, and interdisciplinary team collaboration.
Must be a Licensed Registered Nurse (RN) with a valid driver's license and reliable vehicle. Proficiency in Electronic Medical Records (EMR) software and strong organizational skills are required.
Conduct initial client assessments and continuously refine care plans. Provide hands-on nursing care and patient education. Maintain open lines of communication with patients, families, and the healthcare team. Manage patient care coordination for your caseload. Participate in interdisciplinary team meetings to ensure comprehensive care planning.
Licensed as a Registered Nurse / RN in the state you work in. Proficiency with Electronic Medical Records (EMR) software. Excellent planning, organizing, and communication skills. Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards. Valid Driver's License and reliable vehicle with current insurance
