Licensed Practical Nurse (LPN) - Night & Weekend Day Shifts

Position Summary

SmithLife Homecare is seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our team to provide overnight and weekend nursing support for clients in their homes. In this role, the LPN will provide skilled nursing care, medication management, and ongoing client monitoring to ensure safety, comfort, and continuity of care.

Working under the supervision of a Registered Nurse (RN), the LPN will implement individualized care plans, monitor client conditions, administer medications, and support clients and families throughout the care journey.

• Key Responsibilities
• Deliver skilled nursing care in accordance with the established Plan of Care
• Provide medication management and monitoring to ensure medications are administered safely and on schedule
• Administer medications, injections, and treatments as prescribed
• Monitor and document client condition, including vital signs and response to care
• Observe and report changes in physical, emotional, or cognitive status
• Provide overnight monitoring and supervision to ensure client safety and well-being
• Assist with clinical procedures including wound care and infection prevention
• Support clients with activities of daily living as needed
• Educate clients and families on care needs and disease management
• Maintain accurate, timely documentation in accordance with agency standards
• Collaborate with RNs, care coordinators, and interdisciplinary team members
• Follow all infection control protocols and agency policies

Qualifications

• Active LPN license in Maryland and/or Washington, DC (in good standing)
• Graduate of an accredited practical nursing program
• Minimum 3 years of clinical nursing experience preferred
• Strong clinical judgment, communication, and documentation skills
• Experience in home care, geriatrics, or chronic disease management preferred
• Ability to work independently and adapt in a home-based setting

• Physical Requirements
• Ability to lift, assist, and reposition clients as needed (50+ lbs)
• Frequent standing, walking, bending, and kneeling
• Full use of hands and arms for clinical tasks

Schedule & Status

PRN / Per Diem (As Needed) Shift availability includes overnight, and weekend day shifts based on client care needs and staffing requirements.

We are currently seeking LPN coverage for the following schedules

• Monday –
• Thursday: 8:00 PM – 8:00 AM (Night Shift)
• Friday –
• Sunday: 8:00 PM – 8:00 AM (Night Shift)
• Saturday –
• Sunday: 8:00 AM – 8:00 PM (Weekend Day Shift)

• Competitive hourly rate based on experience
