Provide care management and therapeutic interventions for individuals with severe mental illness transitioning from in-patient settings to the community. Collaborate within a multi-disciplinary mobile team to conduct assessments and develop crisis management plans.
Requires a Master's level license (LCSW, LMHC, or LMSW) and 2-4 years of experience in individual therapeutic counseling. A valid NYS Driver License and reliable transportation are mandatory.

$59,789.34 with supervision.
CTI is a time limited, evidence-based care management approach focused on continuity of care for individuals with severe mental illness during times of transition from an in-patient setting back into the community. The Mental Health Professional (MHP) will be a part of a unique project that partners Unity of House of Troy in Rensselaer County and Mohawk Opportunities in Schenectady County together to provide a multi-disciplinary team approach to supporting participants. The MHP will provide care management services as well as therapeutic intervention, psychoeducation, developing crisis intervention/management plans, skill development, and other clinical intervention as needed.
Provide psychoeducation and care management services to individuals across both Rensselaer County and Schenectady County CTI participants.
Work as a part of a multi-disciplinary mobile care management team.
Complete person-centered care planning.
Conduct mental health assessments.
Provide therapeutic intervention for both mental health and substance use disorders.
Provide a range of treatment, rehabilitation, and support services.
Develop and maintain relationships with community partners.
Complete required paperwork and administrative tasks in a timely and efficient manner.
Promote overall wellness and self-management.
Promote a culture of inclusion and belonging.
