Coordinate and manage patient referrals for Valve and Structural Heart procedures, including scheduling, diagnostic testing, and intake documentation. Act as a liaison between the multidisciplinary team and patients to ensure a seamless transition from pre-op to post-op care.
Requires a BSN or ADN and a valid RN license, along with BLS certification. Candidates must have at least 5 years of clinical medicine experience or 3 years of specific cardiology experience.
This is a hybrid role that requires ~2 days per week onsite at CMC and the remainder of the week you will follow the physician to clinic or work from home (depending on physician/dept schedule). Flexible day time hours 5 days a week (Mon-Fri) and the ability to stay late on occasion of clinic runs late. Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Education (BSN) Bachelor's Degree in Nursing (Required) or (ADN) Associate's Degree in Nursing (Required) Work Experience 5+ years In clinical medicine (Required) or 3+ years Cardiology experience (Required) Ambulatory nursing experience (Preferred) Knowledge, Skills, & Abilities Current knowledge of cardiovascular disease management, trends, care, etc. across the different practice settings. (Preferred proficiency) Understanding of hospital discharge process; i.e. DME, home care, skilled nursing, etc (Required proficiency) Self-directed and motivated (Required proficiency) Detail oriented (Required proficiency) Strong communication skills (Required proficiency) Familiar with Microsoft Office (Required proficiency) Licenses and Certifications Registered Nurse (RN), Ohio and/or Multi State Compact License (Required Upon Hire) and Basic Life Support (BLS) (Required) Advanced Cardiac Life Support (ACLS) May be required based on clinical area standards. Pediatric Advanced Life Support (PALS) May be required based on clinical area standards.
