Key Responsibilities Deliver patient treatment as directed by the Physical Therapist in accordance with physician orders.
Accurately complete daily treatment notes and weekly progress documentation per PT Board standards and facility guidelines.
Participate in interdisciplinary care planning, rehab conferences, and team communication.
Assist in training Restorative Aides in collaboration with the nursing team to promote continuous patient mobility.
Educate residents, families, and caregivers on maintenance programs and safe discharge planning.
Maintain professional relationships and ensure all rehab equipment remains in excellent working condition.
