We are seeking a dedicated Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our collaborative outpatient team.

At H2 Health Dublin, you'll work with a diverse patient population that includes orthopedic conditions, post-surgical rehabilitation, sports-related injuries, balance and mobility concerns, and functional movement impairments across pediatric, adult, and geriatric populations.

Your Role Physical Therapist (PT) Evaluate and develop individualized treatment plans for patients with orthopedic and musculoskeletal conditions.

Deliver evidence-based physical therapy interventions to help patients achieve their functional goals.

Collaborate with physicians, referral sources, patients, and caregivers to ensure coordinated care.

Monitor patient progress and modify treatment plans as appropriate.

Maintain timely and accurate documentation using technology designed to streamline clinician workflows.

Physical Therapist Assistant (PTA) Implement treatment plans established by the supervising Physical Therapist.

Provide therapeutic exercises, manual techniques, functional training, and patient education.

Monitor patient response to treatment and communicate progress to the Physical Therapist.

Assist patients in achieving improved mobility, strength, function, and independence.

Maintain accurate documentation and support an exceptional patient experience.

Physical Therapist (PT) Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program.

Current Virginia Physical Therapist license or eligibility for licensure.

Strong communication, interpersonal, and clinical decision-making skills.

Passion for delivering patient-centered outpatient rehabilitation services.

Physical Therapist Assistant (PTA) Associate degree from a CAPTE-accredited Physical Therapist Assistant program.

Current Virginia Physical Therapist Assistant license or eligibility for licensure.

Strong communication and teamwork skills.
