Provide essential personal care, assistance with daily living activities, and health monitoring for elderly participants in their homes and at the PACE Center. Maintain a safe environment through housekeeping and transport participants for errands while reporting clinical changes to staff.
Must be at least 18 years old with a high school diploma and a current State Registered Nurse Aide certification from the Kentucky Board of Nursing. Requires a valid driver's license, auto insurance, CPR certification, and at least one year of experience with the frail and elderly.

The PACE Home Care CNA plays a vital role in supporting the health and well-being of participants enrolled in the Program of All-Inclusive Care for the Elderly (PACE). This position is responsible for providing essential personal care and assistance with daily living activities both in the participants’ homes and within the PACE Center, as needed. The PACE Home Care CNA duties include assisting with bathing, dressing, toileting, meal preparation and feeding, monitoring vital signs, and documenting intake and output. The PACE Home Care CNA helps maintain a clean, safe, and supportive environment through routine housekeeping tasks and chore services. Additional responsibilities include transporting and assisting participants with simple errands. A CNA in this role requires compassion, attentiveness, and a strong commitment to delivering quality care while observing and reporting changes in participants’ conditions to the appropriate clinical staff.
Is at least 18 years of age and has a High School Diploma or General Education Degree (GED). Is currently a State Registered Nurse Aide through the Kentucky Board of Nursing. Proof of completion of recognized CNA training program. Must have at least one year of experience working with the frail and elderly or proper training upon hire. Food handling permit, to be completed upon hire. Possesses and maintains current CPR certification. Is self-directed with the ability to work with little direct supervision. Demonstrated knowledge and skills necessary to provide care to and communicate with the adult population. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to work collaboratively with PACE staff and members of the Interdisciplinary Team. Ability to speak effectively before groups of participants or team members. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized solutions. Ability to adapt to participant status and needs as directed. Ability to accommodate schedule changes daily due to staffing, participant condition and needs, and new participants. Ability to follow emergency and evacuation procedures. Must be a licensed driver with automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage. Demonstrates tact, patience, and good personal hygiene. Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
