Deliver high-quality medical and emotional care to assisted living and memory care residents, including medication management and oversight of nursing assistants. Coordinate with healthcare providers and families while assisting residents with activities of daily living.
Requires a valid Licensed Practical Nurse (LPN) license and a high school diploma. Candidates should have basic nursing experience and the ability to perform medical assessments and medication administration.

Deliver the highest quality care to our assisted living and memory care residents by providing oversight of their medical and emotional needs by recognizing medical issues and contacting the appropriate healthcare providers, family members, and/or responsible parties, implementing medical orders, managing and distributing medications as well as assisting with activities of daily living.
and Tasks: 1. Consistent and predictable attendance. 2. Dispense prescribed medications to residents according to doctors' written orders. Document properly. 3. Oversee medical care of assisted living residents: a. Act as shift lead for nursing assistants and med techs on a shift. b. Recognize medical issues as well as monitor the status of chronic illnesses. c. Coordinate with external health care providers such as home health and hospice. d. Administer health assessments such as LOC, UAI and ISP. e. Communicates any concerns to healthcare providers as well as family members or responsible parties. 4. Document daily including, but not limited to, medication administration, nursing notes, communication with healthcare providers and family members/responsible parties plus complete orders. 5. Provide direct personal assistance and supervision of residents ADLs when necessary, including, but not limited to, bathing, dressing, eating, walking, etc. Assist residents with any personal needs that the resident may be unable to do themselves. 6. Respond immediately in case of an accident or incident involving a resident. Document incident. 7. Attend training as required as well as complete all Relias training as scheduled.
