Provide skilled, one-on-one nursing care to patients in their homes and develop individualized care plans. Collaborate with interdisciplinary teams and maintain accurate documentation in compliance with regulatory requirements.
Must hold a current Colorado RN license, CPR certification, and a valid driver's license with a reliable vehicle. Home health experience is preferred, and candidates must pass pre-employment screenings.
