Provide skilled, patient-centered care in home settings, including comprehensive assessments and the development of individualized care plans. Coordinate with physicians and interdisciplinary teams to manage chronic conditions and prevent hospitalizations.
Requires a graduate degree from an accredited RN program and an active South Carolina or Compact RN license. A minimum of one year of nursing experience is required, with preference for home health or critical care backgrounds.
