Facilitates resource management and coordinates the transition of inpatients from the hospital to home. Provides psychosocial assessments and interventions to ensure continuity of care and cost-effective resource utilization.
Requires a BS degree in Social Work and current Alabama licensure. Two years of hospital experience or a comparable combination of education and experience is preferred.
Overview Facilitates resource management activities for inpatients based on identified patients. Coordinates patient’s transition from hospital to home through collaboration with physicians, nursing staff and other healthcare professionals to enhance continuity of patient care, promote cost effective utilization of resources and enhance patient, nurse and physician satisfaction. Provides psychosocial assessment, resources and intervention to hospitalized patients.
Education: Require BS degree in Social Work from an accredited college or university and current Alabama licensure.
