Compensation

$41.50 – $50.00 per visit + mileage reimbursement
Schedule: Full-Time Hours Monday - Friday

What You’ll Do

Provide in-home visits supporting patients and families.
Deliver counseling, care planning, and crisis intervention.
Connect patients to community resources and support systems.
Collaborate with an interdisciplinary care team.

What You Bring

Master’s Degree in Social Work (MSW) required.
Active LGSW or LISW license in Minnesota.
At least 1 year of clinical Social Work experience.
A passion for independent, relationship-based care.
