Min USD $45,000.00/Yr. Max USD $50,003.20/Yr. Position Overview The Case Manager

Provides and coordinates client services for Harmonia Family Shelter. This position is located at East 31st street Manhattan Essential

Duties & Responsibilities

Conduct intake process with new clients within 24 hours of admission; includes but is not limited to explanation of residents rights and responsibilities, and grievance procedures. Conduct psychosocial, vocational/educational/educational, and housing assessments and documentation. Provide accurate, complete, and timely documantation of provision of supportive social services and client progression/regression toward the goals of securing permanent affordable housing Prepare housing applications and assist clients with housing applicatio/interview process. Monitor and assess client progression/regression in behaviot and/or appearance. Facilitate group and individual counseling, workshops, psych education, and recreational activities Conduct room /dorm visits/inspections to assess client personal hygiene, room cleanliness, andoverall ADL skills. Provide comprensive case management services to an assigned caseload. Develop initial service plans and conduct service pl;an reviews as needed for an assigned caseload. Provide referrals for in-house and community based medical, mental health, and supportive social services to assigned caseload as appropriate. Ensure that clients attend meals and maintain appropriate nutritional intake. Assist in building evacuation durinf fire drills and building emergencies. Minitor client medication compliance and collaborate with nursing staff . Refer vclients to appropriate community resources as needed. Maintain CPR/First Aid Certification. Maintain up to date, accurate, complete, and timely client chart documantationin compliance with DHS, OTDA. Facilitae discahrges & transfers. Attend staff meetinngs, case conferences, group supervision, trainings and escort clients to housing interviews. Comply with all SUS employee polisies & procedures. A work schedule that includes some weekends & evening hours is required. Requirements

GED or HS Diploma is required. Experience working with chronically homeless individuals or families. Ability to work independently and as part of a team, to individuals of diverse ethnicities, backgrounds, ages, and sexual preferences. Two yeasr experienc with homeless population.. Ability to work with a culturally diverse population. Familiarity with NYC public school system preferred. Experienc in residential setting, group work, case management and community outreach.
