The Registered Nurse Case Manager is responsible for admitting patients, developing patient-specific plans of care, and coordinating services with providers. They oversee LPNs and Home Health Aides while ensuring accurate OASIS documentation and safe delivery of care in the home.
Candidates must have a Nursing Diploma or Associate Degree (BSN preferred) and at least two years of acute care nursing experience. A valid Connecticut RN license, driver's license, car insurance, and CPR certification are required.
