$28 - $33 / HOUR
Perform daily street outreach to engage homeless adults and connect them to primary medical care, psychiatric services, and social benefits. Provide health education, harm reduction interventions, and transport participants to essential appointments.
Requires a BA degree in a human services field or CADAAC certification, along with 2 years of experience working with homeless populations or those with co-occurring disorders. A valid California Driver's License, personal vehicle, and flexible schedule for evenings and weekends are mandatory.
Abode, one of the largest and effective nonprofits working to end homelessness in the Bay Area, is seeking a Community Health Outreach Worker for our programs in Alameda County.
The Community Health Outreach Worker will provide linkage, referral and follow along support to connect participants to primary care and ongoing medical and social services. This position will provide street outreach to engage homeless adults in medical care, apply for benefits, referral to psychiatric care, social services, and complete coordinated entry assessments.
