This travel - temporary assignment is for a PT who will work in an outpatient department within a hospital setting, with primary emphasis on outpatient therapy and a smaller component of LTC/Skilled Swing tasks. The assignment runs for 13 weeks with a strong possibility for extension. Housing is not included; a list of housing options can be provided upon request.

Responsibilities
Deliver individualized physical therapy services in an outpatient hospital setting, including assessment, treatment planning, implementation, and progress monitoring
Develop and implement customized treatment plans to maximize functional outcomes and return to activity
Educate patients and families on exercise programs, self-management, and safety to support ongoing progress
Reassess patient progress regularly and adjust plans to meet functional goals
Document evaluations, treatment plans, progress notes, and outcomes clearly in the medical record
Collaborate with interdisciplinary team members to coordinate care and ensure consistent communication
Requirements
Availability to work a 40-hour week, Monday through Friday, 8:00 am–5:00 pm
Ability to perform PT duties in an outpatient hospital setting with occasional LTC/Skilled Swing tasks
Strong communication and documentation skills
Comfort working within a collaborative multidisciplinary team
Eligibility to participate in a 13-week travel assignment and to travel to Baker, MT for the duration of the contract
