Responsibilities

Conduct psychosocial assessments during patient admission Evaluate social, emotional, and environmental factors impacting care Collaborate on interdisciplinary care plans Provide counseling, resource linkage, and discharge planning support Consult with physicians and care teams regarding patient needs and care adjustments Maintain timely and accurate clinical documentation Supervise Social Work Assistants as needed

Requirements
