The Patient Navigator coordinates access to the full continuum of cancer care, from screening to post-treatment resources. They collaborate with interdisciplinary teams to connect patients with internal and community resources such as financial support, transportation, and medication assistance.
A Bachelor's degree in Social Work, Psychology, Sociology, Family and Consumer Science, or a related field is required. Preference is given to candidates with 3-5 years of clinical experience in healthcare or social services settings.
