What You’ll Do

Complete Start of Care (SOC) admissions in patients’ homes.
Conduct detailed clinical assessments and OASIS documentation.
Initiate care plans and ensure a smooth transition into services.
Educate patients and families on what to expect moving forward.
Coordinate with the care team to set patients up for success.

What You Bring

Active Minnesota RN license.
Experience with home health admissions and OASIS preferred.
Strong time management and independent work style.
