We are hiring an LPN for our Population Health Division! Monday - Friday 8 hour day shifts Hybrid 3 days a week after 90 days working independently NO NIGHTS OR WEEKENDS Position can be located at our Primary Care Clinic in Independence, MO or Platte City, MO Our Population Health Division is hiring an experienced LPN that is looking to grow in a new position with high visibility. This position would allow you to work independently and plan out your own day. As an Outreach Specialist you would coordinate care for our low to moderate risk patients after they have been discharged from the hospital through transitions of care follow-up calls. This position requires you to have clinical knowledge as an LPN. As an Outreach Specialist you will have the opportunity to use your LPN background to help optimize health management for a portion of our patient population by calling patients and playing an integral role in impacting the care we are able to provide our patients.
