The coordinator performs patient follow-ups and evaluations to ensure adherence to physician orders and provides essential patient education. They are also responsible for delivering enteral feeding equipment and supplies directly to patients' homes or businesses.
Preferred candidates have one year of experience in home health, hospital, or clinic settings, specifically as an LPN, DTR, or CNA. Education in Dietetics, Nutrition, Home Health, or Nursing is strongly preferred.
This employee performs patient followup, generally responsible for the evaluation of patient adherence to physician orders.
The employee must occasionally lift and/or move up to 50 pounds.
