This individual will serve as a liaison between participant and medical staff. Clinic Scheduler is responsible for greeting and directing participants, checking participants in and out of the clinic, answering the telephone, data entry of patient demographic information, scheduling and confirming clinic appointments and other administrative duties in support of the organization’s operations.

• Duties include, but are not limited to: Collaborate with care team members specifically the Primary Care Physician (PCP) and Registered Nurse Case Manager (RN CM) for appropriately scheduling and rescheduling participants’ clinic appointments Scheduling participants’ assessments. Answer the telephone by the third ring in a friendly, professional manner. Assist with coordination of day-to-day front office check-in/out operations. Use customer service principles and techniques to assist with participants, physicians and medical office staff in a professional, calm and pleasant manner. Responsible for confirming and documenting participant’s appointments in the Electronic Health Record (EHR). Ensure participants are aware of upcoming appointments and are reminded on a regular basis to ensure appointments are kept or rescheduled in a timely manner. Identify issues that may prevent a participant from attending scheduled appointments; and communicating solutions to management and participant. Review schedules and identify challenges and develop a plan for resolution. Pulling files, determining multi-level scheduling needs for maximum participant engagement and satisfaction. Ensure necessary equipment or supplies are available for participants scheduled appointments. Demonstrate an understanding of participant confidentiality and rights to protect participant and business information. Thorough knowledge of current concepts and practices of health and social services to the elderly, particularly of home- and community-based services within an interdisciplinary team facilitation model. Knowledgeable as an advocate for risk-based long-term care programs, specifically the PACE model. Ability to work effectively and harmoniously with staff, elderly, public and private agencies, governmental officials, providers of services, and peers. Energetic, well organized, dependable, flexible, and resourceful. Effective oral and written communication skills. Attend and participate in staff meetings, in-services, projects, and committees as assigned. Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance. Accept assigned duties in a cooperative manner; and perform all other related duties as assigned. Be flexible in schedule of hours worked. May be required to use personal vehicle, if applicable. If using a personal vehicle, a valid California Driver’s License is required. QualificationsKnowledge, Skills and Abilities Proficient knowledge of computer skills. MS Office (Word, Excel, Access, PowerPoint, and Outlook). Knowledge of general office procedures, equipment and filing systems. Ability to communicate effectively, both orally and in writing. Ability to quickly learn department policies, procedures, goals, and services.
