Provides comprehensive pre- and post-transplant pharmaceutical care for abdominal transplant recipients within a multidisciplinary team. Responsibilities include monitoring drug therapies, developing clinical protocols, and educating patients and healthcare staff.
Requires a Doctor of Pharmacy degree, a valid California pharmacy license, and BLS certification. At least three years of acute care experience is required, with PGY1/PGY2 residency training in Transplant Pharmacy preferred.
The Clinical Pharmacist Specialist, Transplant Services provides pharmaceutical services to the hospital’s patients, leading in the delivery of pharmaceutical care for transplant recipients. Provides pre- and post-transplant clinical pharmacy services. All associated areas may be included as well, including transferring units, procedural areas, and outpatient clinics.
Performs other duties as assigned.
