Conduct comprehensive occupational therapy evaluations in the patient’s home to assess functional limitations, safety risks, and ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs). Identify physical, cognitive, environmental, and psychosocial factors impacting patient independence. Educate patients, families, and caregivers on therapy goals, home exercise programs, safety precautions, and strategies to promote independence. Monitor patient progress and adjust treatment plans to ensure optimal outcomes. Collaborate with a multidisciplinary healthcare team to coordinate care. Maintain accurate and timely documentation. Coach and support caregivers.

Qualifications: Active Occupational Therapy License (Arizona) Valid Driver's license Reliable transportation
You live in Phoenix or the East Valley (Chandler, Gilbert, Mesa, etc.) You have prior home health experience
