Position Details

• Full-Time: 40 hours per week
• Schedule: Mondays-Fridays 8 or 9am to 5pm with flexibility, and a weekend rotation.
• Team: Community Supported Living (CSL)

• The Community Supported Living Caregiver offers support to individuals with disabilities, who are aging or have mental illness as they strive for independence.
• They provide individualized support, problem solving, companionship, and guidance to individuals in their homes.
• Our Community Supported Living program is designed to help clients maximize their independence and reside in their homes within the community.
• We offer support with chores, housekeeping, wellness, transportation, community engagement and more!

• What you’ll do
• Offer individualized support, problem-solving, and companionship
• Assist with daily living skills (cleaning, hygiene, community engagement)
• Provide medication monitoring (non-prescriber)
• Encourage independence and meaningful community participation
• Foster health and safety through guidance and support
• Complete documentation and attend team meetings
• Participate in ongoing training and development

Requirements

• High school diploma or GED required
• Valid driver’s license & reliable transportation
• Ability to meet auto insurance requirements
• First Aid & CPR (preferred; may be required after hire)

TRAVEL

Daily travel may be required.
