Hospice Registerd Nurse

University Home Care Inc is seeking a compassionate and dedicated Hospice Registered Nurse to join our team in Troy, MI. This role involves providing high-quality, patient-centered care to individuals facing life-limiting illnesses, ensuring comfort and dignity in their final stages of life.

Responsibilities

• Provide comprehensive nursing care to hospice patients in their homes or care facilities, focusing on pain management and symptom control.
• Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team.
• Monitor patient health and progress regularly, adjusting care plans as necessary.
• Offer emotional support and education to patients and their families regarding end-of-life care.
• Ensure accurate and timely documentation of patient care and compliance with regulatory requirements.
• Coordinate with physicians, social workers, and other healthcare professionals to deliver holistic care.

Requirements

• Valid Registered Nurse (RN) license in the state of Michigan.
• Previous experience in hospice or palliative care preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate and patient-focused approach to care.
