Contribute to the assessment, planning, implementation, and evaluation of patient care for seniors with multiple chronic conditions. Work within a multidisciplinary team of physicians, nurses, and pharmacists to deliver innovative quality care.

About the Unit

At Advanced Primary Care, we specialize in patients who are 65+ with multiple chronic conditions like diabetes, hypertension, heart failure and COPD. Join our multidisciplinary team consisting of physicians, APPs, nurses, pharmacists, social workers and MAs. We collaborate together to deliver innovative and quality care.

About Outpatient Clinics

Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.

Scope of work

Under the direction of the RN / Mid-level Provider / Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care.

Qualifications

  • Required High School Diploma or equivalent
  • Required Vocational/Technical Graduate of an approved nursing program
  • Successful completion of competency based orientation program Required
  • Healthcare experience Preferred
  • LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required or CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required

Requirements

Requires a High School Diploma, graduation from an approved nursing program, and a valid LPN license for the State of Michigan. BLS certification from AHA or ARC is required within 90 days of hire.
