Coordinates the discharge process and conducts assessments for patients and caregivers to ensure successful transitions from hospitalization to outpatient care. Identifies post-discharge risk factors and implements interventions to prevent undesirable patient outcomes.
Requires a Master's degree in Social Work from a CSWE-accredited program and licensure by the South Carolina Board of Social Work Examiners. Certification in case management is preferred.

This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
