Provide essential personal care, hygiene assistance, and mobility support to hospice patients in their homes and care facilities. Monitor patient conditions and maintain accurate documentation to ensure comfort and dignity during end-of-life care.
Must hold a current Oklahoma Certified Home Health Aide (CHHA) certification and possess a valid driver's license with reliable transportation. Previous experience in hospice, home health, or as a CNA is preferred.
Full-Time Competitive Pay Mileage Reimbursement Flexible Scheduling Supportive Clinical Team Benefits Full-time employees are eligible for: Medical Insurance Dental Insurance Vision Insurance Paid Time Off (PTO) Holiday Pay Mileage Reimbursement Responsibilities As a Hospice Aide, you will: Provide personal care including bathing, grooming, dressing, and hygiene assistance Assist patients with mobility, transfers, ambulation, and positioning Support patients with meal preparation and feeding as needed Assist with toileting and elimination needs Monitor and report changes in patient condition to the clinical team Record vital signs, intake/output, and other observations as directed Perform light housekeeping tasks related to patient care Maintain accurate and timely documentation of patient visits Participate in care conferences and team meetings as assigned Promote patient comfort, dignity, and safety at all times Qualifications Current Oklahoma Certified Home Health Aide (CHHA) certification required Previous hospice, home health, CNA, or direct patient care experience preferred Valid driver's license Reliable transportation and current auto insurance Strong communication and interpersonal skills Ability to work independently in patient homes Compassionate, dependable, and patient-centered approach to care Physical Requirements Ability to lift up to 50 pounds Ability to assist with patient transfers and mobility Ability to travel throughout the service area Why Join Haven?
