The Case Manager serves as a guide and advocate for justice-involved individuals, facilitating 1:1 and group sessions focused on life skills and recovery. They collaborate with a multidisciplinary care team to monitor progress and connect participants with essential community resources.
Candidates must have a high school diploma, a Class E driver's license, and experience working with vulnerable populations or lived experience in recovery. A Certified Peer Support credential must be obtained within the first 90 days of employment.
