The Care Coordinator manages the patient discharge process from admission through hospital exit, coordinating with interdisciplinary teams to establish individualized care plans. They are responsible for placing referrals for medical equipment and facilities while ensuring seamless transitions to community providers.
Candidates must be a graduate of an accredited nursing program with a current Texas or Compact nursing license and BLS certification. A Bachelor's degree in Nursing and two years of experience in an acute care hospital setting are preferred.

Job Summary Discharge planning begins the day of admission. The job of a care coordinator (CC) is to initially identify the patient's d/c goal and meet with the patient/family to complete a thorough discharge planning evaluation. The CC follows the patient through the hospital continuum and works with the interdisciplinary teams to establish a discharge plan. The CC places referrals for DME, facilities, infusion companies, etc to meet the individualized d/c needs of the patient.
