Provide state-funded case management and behavioral health services for youth with severe emotional disturbances. Responsibilities include conducting diagnostic assessments, developing treatment plans, and coordinating care with multidisciplinary teams.
Requires a bachelor's degree in a human services field, or a combination of a high school diploma with significant clinical experience or psychiatric rehabilitation certification. A valid driver's license and specific certifications like CPI and IM+CANS are required.
Provides case management services, including assessment, treatment planning, and ongoing support for clients with behavioral health needs Conducts diagnostic assessments and develops individualized treatment plans Coordinates care and collaborates with multidisciplinary teams, including providers, social workers, and nursing staff Delivers counseling, crisis intervention, and community-based support to clients and families Monitors client progress and adjusts treatment plans based on clinical findings Maintains accurate, timely clinical documentation in compliance with regulatory standards
Education: Requires a high school diploma or GED and a minimum of five years documented clinical experience in mental health or human services; OR An individual possessing a certificate of psychiatric rehabilitation from a DHS-approved program, plus a high school diploma or GED, plus two years' documented experience in provided mental health services; OR An individual possessing a bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or a related human service field. License(s)/Certification(s): Valid driver’s license. (CPI) training within 6 months of hire. Obtain certification within 2 months of hire and re-certify annually according to the requirements set forth in the Illinois Department of Healthcare and Family Services related to the Illinois Medicaid Comprehensive Needs & Strengths (IM+CANS) *Specific offers are determined by various factors, such as experience, skills, internal equity, and other business needs.
