Implement comprehensive case management and advocacy services for youth aged 18-24 using the Housing First model. Responsibilities include conducting assessments, securing rental agreements, and helping clients achieve long-term housing stability and self-sufficiency.
Requires a high school diploma and at least one year of related work experience, with higher grades available for those with associate or bachelor degrees. Must possess a valid driver's license, insured vehicle, and pass comprehensive background checks.

Job Title: Youth Rapid Rehousing Case Manager
Reports To: Housing First Manager
Under the supervision of the Housing First Manager, this position is responsible for the implementation and delivery of comprehensive case management and advocacy services for individual youth people ages 18-24, and their families participating in the Rapid Re-Housing Program. This position provides services based on the Housing First Model of care with motivational interviewing, strengths-based goal setting, and trauma-informed care approach to work collaboratively with individuals and families towards the general goals of maintaining housing, increasing family stability, in providing family functioning and increasing the self-sufficiency in the program.

1. Must meet minimum requirements above 2. AA Degree in a related field plus two years experience, or BA Degree in a related field or comparable combination of education and experience
