Housing rehabilitation qualified professional (Qp)

📍 Jacksonville, NC 💰 $24.50 per hour 🕒 Full-Time | Monday–Friday

Help individuals build stability, independence, and hope

Pyramid Healthcare is seeking a compassionate and dedicated Housing Rehabilitation Qualified Professional (QP) to join our team in Jacksonville, NC. In this role, you will support individuals living with serious mental illness by helping them obtain and maintain safe, stable housing while promoting recovery, independence, and community integration.

As a key member of the treatment team, you will collaborate with clients, community partners, landlords, and healthcare professionals to ensure individuals have access to the resources and support needed to achieve long-term housing success.

What you'll do

Assist clients in locating, securing, and maintaining appropriate housing opportunities.

Provide housing rehabilitation and supportive services that promote independent living skills.

Advocate for clients with landlords, housing authorities, and community agencies.

Coordinate housing-related services and resources to address barriers to stable housing.

Collaborate with multidisciplinary treatment team members to develop and implement person-centered treatment plans.

Monitor client progress and maintain accurate, timely documentation in accordance with agency and regulatory requirements.

Connect clients to community resources that support recovery, wellness, and self-sufficiency.

Educate clients on tenant responsibilities, budgeting, and housing retention strategies.

Required qualifications

Bachelor's Degree in Social Work, Psychology, Counseling, Vocational Rehabilitation, Human Services, or a related field from an accredited college or university.

Qualified Professional (QP) status according to 10A NCAC 27G .0104.

Experience working with individuals diagnosed with serious and persistent mental illness preferred.

Knowledge of community housing resources, supportive housing programs, and rehabilitation services.

Strong communication, advocacy, and relationship-building skills.

Ability to work independently while collaborating effectively within a multidisciplinary team.

Valid driver's license and ability to travel within the service area as needed.

Preferred Qualifications

  • 1–3 years of experience providing rehabilitation, case management, housing support, or community-based behavioral health services.
  • Experience working with housing authorities, landlords, and community agencies.
  • Familiarity with recovery-oriented and person-centered treatment approaches.
