• What You'll Do
• Manage a caseload of hospice patients in homes, assisted living communities, and skilled nursing facilities
• Coordinate care with physicians and the interdisciplinary team
• Deliver expert symptom management and clinical oversight
• Educate patients and families throughout the hospice journey
• Ensure accurate and timely documentation

Qualifications

• Active Georgia RN license
• Valid driver's license and reliable transportation
• Minimum one year of nursing experience
• Hospice or home health experience preferred
