Description BMS Family Health and Wellness Centers is seeking a compassionate, experienced, and dedicated Nurse Practitioner – Adult Medicine to join our growing team in Brooklyn, NY. The Nurse Practitioner will provide comprehensive primary care services to adult patients, including preventive screenings, health assessments, diagnosis, treatment, and ongoing management of acute and chronic medical conditions. As a key member of our clinical team, you will work collaboratively with physicians, nurses, and other healthcare professionals to ensure exceptional patient care. Key Responsibilities Conduct comprehensive health assessments, screenings, and physical examinations for adult patients. Diagnose and treat acute, chronic, and preventive healthcare needs. Develop and implement individualized treatment plans based on clinical findings. Provide patient education and counseling to promote health and wellness. Coordinate care with specialists and other healthcare providers as needed. Participate in regular chart reviews and quality improvement activities. Maintain an active clinical practice consistent with professional standards and BMS policies. Collaborate with the medical team to ensure continuity and quality of patient care. Support compliance with Managed Care/HMO requirements, SBHC contracts, and Federal and New York State healthcare regulations. Contribute to the development and implementation of clinical policies and procedures. Regularly consult with clinical leadership, including the Assistant Medical Director and Nurse Manager, regarding patient care and clinical operations.
Required: Current New York State license as a Nurse Practitioner. Minimum of two (2) years of clinical experience as a Family Nurse Practitioner or Adult Medicine Nurse Practitioner. Strong clinical assessment, diagnostic, and patient management skills. Excellent communication and interpersonal abilities.
