Coordinate services and connect individuals experiencing homelessness to community resources to achieve housing and recovery goals. Develop person-centered recovery plans and provide direct support for housing stability and independence.
Requires a minimum of 2 years of experience in behavioral health, substance use, or IDD services and a valid driver's license. A bachelor's degree in Human Services or a related field is preferred.

RHD Montgomery County Homeless Services offers a safe place to stay and assistance for individuals experiencing homelessness, helping them transition towards independent living. This is a new site located in Lansdale, PA, and the program specifically caters to single adults without housing. Clients have access to dedicated caseworkers who help them find permanent housing and apply for necessary benefits. Additionally, we provide practical support such as laundry facilities, transportation, meals, clothing, and essential life skills training.
This program provides a safe environment and comprehensive support services to help individuals transition to stable, independent living. Services include case management, housing support, life skills training, and access to essential resources.

The Case Manager plays a critical role in supporting participants at Genny’s Place by coordinating services, connecting individuals to community resources, and helping them achieve their recovery and housing goals.
This role requires a compassionate, proactive professional who can advocate for participants, manage multiple responsibilities, and work collaboratively within a multidisciplinary team.
