Position Overview We are seeking a passionate and patient-centered Physical Therapist (PT) to join our growing team. In this role, you will deliver high-quality therapy services to residents in assisted living environments, helping them restore mobility, function, and confidence. Key Responsibilities Provide treatment visits, 30-day reassessments, re-certifications, and discharge evaluations Develop and implement individualized treatment plans with measurable short- and long-term goals Supervise and mentor Physical Therapist Assistants (PTAs) Manage a full caseload (12–28 visits per week for part-time positions) Maintain accurate documentation using an electronic medical record system Recommend and assist in fabrication of adaptive devices as needed Educate patients, families, and caregivers on therapy techniques and equipment use Collaborate with interdisciplinary healthcare teams to ensure coordinated, high-quality care
Qualifications
