The Licensed Social Worker supports the emotional and social well-being of residents by developing personalized programs and services. Key duties include coordinating care with interdisciplinary teams and serving as a liaison between residents, families, and community agencies.
Candidates must be a Licensed Social Worker with a degree from an accredited school of Social Work. Experience in geriatric or long-term care is strongly desired, and the ability to travel between Nocona and Olney, TX is required.
We are seeking a Licensed Social Worker that will split their time between Nocona Rehabilitation and Care Center and Olney Rehabilitation and Care Center. The Licensed Social Worker plays a key role in supporting the emotional and social well-being of our residents. Under the direction of the Administrator, this role is responsible for planning, organizing, and developing programs and services that meet the medically related emotional and social needs of each resident. The ideal candidate is professional, compassionate, and committed to person-centered care.
