Active Connecticut LPN License New York applicants must maintain active LPN licenses in both New York and Connecticut Minimum of 1 year of pediatric experience in home care or acute care preferred Minimum of 1 year of experience providing pediatric trach and G-tube care Current BLS Certification Authorized to work in the United States Physical Exam completed within the past year PPD/QuantiFERON completed within the past year, or Chest X-Ray completed within the past five years At Affirmed Home Care, we are committed to providing exceptional clinical care while supporting our nurses every step of the way.
